*We will email you a summary report, including key findings and initial evaluation recommendations, at no additional charge. However, if you request a more comprehensive, detailed written report, which includes test scores, we charge $100 for this service.

We are a private pay practice and do not accept insurance.

We will gladly provide a monthly invoice that you can submit to your insurance company for out-of-network reimbursement if applicable. The invoice will show appropriate CPT procedures and diagnostic codes.
